Behavioral Health - Remote in North Dakota - Family NavigatorSupport families across the state seeking access to behavioral health services for children and youth as a Family Navigator for the Behavioral Health Division at North Dakota Health and Human Services (HHS).As a Family Navigator, you will review universal applications and guide families through the process of accessing the right level of care. You will work closely with referral sources, Human Service Centers, and other partners to ensure a smooth and seamless experience for families. You will provide necessary information, facilitate admissions, and serve as a liaison between families, HHS, and other stakeholders. You will also communicate directly with families to explain processes, provide informed consent, gather required information, and assist with care coordination for outpatient services. Additionally, you will help families understand the various care level guidelines and assist in setting up the appropriate services.To thrive in this position, you must be empathetic and dedicated to supporting families through complex processes. You can effectively explain procedures, provide informed consent and clarify information in ways your audience will understand. You are organized and detail-oriented, capable of managing multiple cases while ensuring necessary information is gathered accurately and efficiently. You take initiative in establishing timelines and goals and are proactive and adaptable to changing circumstances while maintaining a family-centered approach. You are a strategic communicator able to identify when to bring in other partners to ensure comprehensive support.To be considered for the Family Navigator position, you must have a bachelor's degree in a human services field, social services, education or closely related field; AND experience in case management, eligibility work, health insurance claims, education working with youth and families or similar experience. In addition, you must have strong Excel skills to include data entry, Pivot tables and the ability to analyze data, identify gaps and make recommendations.About HHS: ND HHS strives to make North Dakota the healthiest state in the nation by reinforcing the foundations of well-being: physical, economic, and behavioral.
