Career Description




The purpose of this position is to assist individuals that may be elderly and/or disabled in their home with the goal of helping them to maintain their independence and quality of life. The primary responsibilities of the Direct Care Associate to help with this goal are as follows:Provide direct care services to individuals that are elderly/disabled in their homesProvide personal care services to include: bathing, hair care/skin care, dressing and toiletingProvide homemaker services to include: light housekeeping, laundry, meal preparation, and assistance with shopping, money management, and communicationProvide companion care servicesProvide non-medical transportationAdhere to the Individual Care Plan developed by the Social WorkerComplete timely and accurate recordsParticipate in required trainingParticipate in monthly team meetings

Minimum Qualifications

Direct Care Associate II: Requires high school diploma or GED and current ND medical certification as a Certified Nurse Assistant (CNA), Documentation of Competency/Quality Service Provider (QSP), or Emergency Medical Technician (EMT), or Licensed Practical Nurse (LPN), or Registered Nurse (RN), or First Responder. Please send a copy of applicable certification with application.Direct Care Associate I Underfill: If no qualified applicants are found at the II level, consideration will be given to underfilling as Direct Care Associate I. Minimum qualifications require a high school diploma or GED. The individual hired at this level will be required to complete the medical certification training at the earliest available opportunity.A medical self-declaration, criminal background check, and a driver's record check may be performed prior to employment. This position will require transportation of clients. Ability to drive, proof of capability to transport clients and adequate vehicle insurance required.
